Some of the terrible covers come down to nothing but a terrible idea with even worse execution, others are down to mishaps (*cough* Okami). Some of the covers are from old systems like the 3DO and others are more recent. It seems that no matter what we do, we will always suffer through the occasional horrid game cover. I will never forgive my Orange Box Cover for causing slight blindness in my left eye. I curse the guy who was responsible for that cover, damn you neon orange man.
